Bibliographic Guide

Did you:

Start a new page and put a heading: BIBLIOGRAPHY?
Put the author's name surname first, then his/her initials or first name?
Take the title from the title page, not the cover?
Include the subtitle if there is one?
Underline the title?
Include the last publishing date (not a reprinting date)?
Put in the correct punctuation (including capital letters)?
List the books in alphabetical order by author's surnames or corporate author, or by title, if there is no author?
Indent the second and third lines (if any)?
